Care Navigator / Social Worker - Finger Lakes Area Counseling & Recovery FLACRA

Job Summary:
The Care Navigator, Social Worker plays a critical role in helping elderly residents access necessary services to age in place. This role involves providing information, processing referrals, conducting needs assessments, coordinating services, and building strong partnerships with community service providers. The Care Navigator ensures residents receive the supportive services they need to maintain their independence and quality of life.
Required Qualifications:
Valid New York State Drivers License.
Bachelors degree in health or human services.
At least 2 years of experience in Health or Human Services, with knowledge and experience in areas such as elderly care, community resources, housing, and employment services.
Availability to work evenings and weekends as program needs require.
Key Responsibilities:
Conduct outreach, enrollment, and initial screenings for supportive service needs.
Partner with the coordinator to assess new residents service needs within 14 days of move-in.
Complete annual needs reviews for all residents in the program.
Provide care navigation support, including assistance with accessing financial support, personal care services, homemaker services, meals-on-wheels, transportation, counseling, visiting nurse services, preventive health screenings, wellness programs, and legal advocacy.
Establish and maintain linkages with community agencies to streamline service workflows.
Communicate and meet periodically with service providers to discuss residents needs and identify strategies for improving access to services.
Serve as the key link between property management and residents, ensuring clear communication and support.
Assist residents in assessing service needs, determining eligibility, and connecting with supportive services.
Skills & Experience:
Strong interpersonal and communication skills to work effectively with culturally diverse residents, staff, and community service providers.
Knowledge of local behavioral health services and substance abuse resources.
Ability to manage multiple tasks in a fast-paced environment while maintaining strong problem-solving, time-management, and organizational skills.
Proficiency in typing (minimum 30 WPM) and using electronic health records (EHR).
Proficient in Internet navigation and Microsoft Office Suite (Outlook, Word, Excel).
Ability to remain calm and provide effective crisis intervention in high-pressure situations.
