Responsibilities:
- Care Coordination: Facilitate communication and collaboration among healthcare team members to ensure coordinated and efficient care delivery.
- Transitional Care Management: Develop and implement comprehensive discharge plans, ensuring patients have the resources and support they need for a smooth transition home or to another care setting.
- Psychosocial Assessment: Conduct thorough psychosocial assessments to identify and address emotional, social, and financial barriers to care.
- Patient Advocacy: Advocate for patients' rights and needs, ensuring they receive culturally sensitive and appropriate care.
- Resource Navigation: Connect patients and families with community resources and support services to address social determinants of health.
- Documentation: Maintain accurate and timely documentation of all patient interactions, assessments, and care plans.
Schedule:
- Full-time, 40 hours per week
- Day Shift
Qualifications:
- Required:
- Master's Degree in Social Work from an accredited program.
- Upon hire: California Clinical Social Worker License.
- Two years of experience in a healthcare-related field (Acute, Ambulatory, or Post-Acute).
- Two years of experience in Acute Care Management (Care Coordination) or successful completion of the Transitions in Practice (TIP) program for Care Managers.
- Preferred:
- Strong experience with discharge planning.
- Current inpatient social work experience.
