DUTIES OF POSITION
Assists in the coordination of client care/services under the direction of the LPN Coordinator, to included fill-ins and scheduling
Staffing coordination duties include the accurate and timely communication of scheduling changes between office and field staff
POSITION RESPONSIBILITIES
Visits clients homes to monitor and reports caregivers interactions,
Documents and reports changes in client status, in service requirements, in and employee staff assignment and in particular, quality of performance
assist with non medical training of Aides
Assists in the scheduling of appropriately skilled person to fill the case, inputs scheduling information on the computer and continues to assist in the coordination of the scheduling
Schedules replacement workers as needed and input scheduling information
Fills in where necessary for open shift where replacement workers are not available, working a minimum of 20hrs
Assists in communication with clients/families on home aide related issues
Answers phones, receives intake information for paraprofessional cases and enter the appropriate information in the computer
Completes intake forms and route them appropriately for admission approval
Assists in the establishment of personnel files and in the checking of references for paraprofessionals being considered for hire
Takes potential employee referral information and passes along referrals as soon as possible
Handles matters related to staff and the client they serve, with the exception of issues that require professional/clinical judgment
Assists in the maintenance of case management notes related to personal problems and/other situations that arise with staff including those relating to client care
Communicates on a routine basis with contract agencies to maintain smooth relations and to obtain needed information
Performs other duties as directed
QUALIFICATIONS
High school graduate ,
Experience in a Home Care setting preferred (HHA, STNA, MA)
Must be computer literate and able maintain simple records in English
Job Types: Full-time, Part-time
Salary: $29,120.00 - $31,200.00 per year
