Position Summary
We are seeking a Care Coordinator to join our team. The Care Coordinator plays a pivotal role within Home Care Placement, overseeing client management, managing new client referrals, in-person client consultations, quality assurance, and staffing of HCAs for the agency. Working alongside the Executive Director, the Care Coordinator ensures adherence to the organization's policies and procedures, implementing corrective action when necessary. Additionally, they are responsible for maintaining compliance with client consultation documents. The role also involves assisting the talent support recruiting and onboaring process. Confidentiality in overseeing client information is imperative and must not breach HIPAA regulations. The Care Coordinator will be responsible for a comprehensive understanding and management of Outlook/Outlook Calendar, Wellsky, and Salesforce.
Minimum Eligibility Requirements
- Bachelor's Degree or equivilant experience preferred
- 1-2+ years of customer service or office experience
- Basic computer knowledge, data entry, word processing
- Excellent customer service, problem-solving and interpersonal skills
- Willingness to adapt and wear many hats in a healthcare startup organization
- Ability to collaborate across departments and work in a fast-paced, team environment
Base Pay is $67,000/year and this is a salaried, exempt position.
The role is full-time, Monday - Friday in our office in San Diego, CA. (University City) The Care Coordinator will participate in an on-call rotation for After Hours / Weekend / Holiday coverage.
Responsibilities
- Ensure that all state and agency requirements regarding certifications are met by each HCA.
- Create and maintain personnel records, and ensure personnel information is up-to-date in personnel record.
- Act as the liaison for the HCAs with the agency and evaluate them on a periodic basis.
- Field inbound referral calls from prospective families, meet with families for an in-person consultation and to build a Care Plan and review expectations.
- Staff cases with competent HCAs who are a good match for the client, based on information gathered from the Care Plan.
- Communicating with clients and families about concerns with HCAs or changes in scheduling and / or staffing.
- Ensure that Organization policies and procedures are being adhered to by all HCAs and implement the appropriate corrective action when necessary.
- Document events in the Organization's Management System.
- Work in conjunction with the Executive Director and administrative support staff to make sure operations in the agency run smoothly.
- Report suspected, observed, knowledge of, reported to, incidents that reasonably appear to be abuse or neglect involving an elder or dependent adult.
- Basic cross-training in other roles of the business, so you can assist as needed and directed by the Administrator.
- Participate in an On-Call rotation to provide 24/7 support to our clients and HCAs for any issues that can arise outside of standard business hours.
