Mid-Columbia Center for Living (MCCFL) is a Certified Community Behavioral Health Clinic (CCBHC) that specializes in providing services that are both trauma-informed and recovery-oriented to persons / consumers with mental illness and addictions. Trauma-informed care includes safety, choice, collaboration, trustworthiness and empowerment for both clients and staff. MCCFL serves Hood River, Wasco and Sherman Counties and utilizes a consumer-involved, consumer-engaged and consumer-driven model. MCCFL provides a whole-person care approach to adults and children to address both physical and mental health needs. MCCFL offers a variety of programs including a mobile crisis unit, intensive case management, ongoing counseling, community support, supported employment, jail diversion, hospital discharges, hearings and more.
Classification General Statement:
The Care Coordinator 1 assists clients in acquiring services needed to achieve treatment goals, with an emphasis on coordination of services both in MCCFL programs and in partner agencies throughout the community. This position regularly interfaces with clients and service providers to ensure a smooth delivery of services in accordance with Oregon statutes and MCCFL policies and protocols.
This position’s care coordination duties are integrated with those of a medical assistant working with behavioral health clients in support of assigned medical providers.

Knowledge: Knowledge of symptoms and challenges faced by people with mental illness; knowledge of modern medical assisting methods and practices; knowledge of assessment and referral processes and procedures; knowledge of medication management; knowledge of MCCFL-based and other community-based services; knowledge of mental health and medical assessment, treatment and service terminology; knowledge of treatment/service planning; knowledge of medical records maintenance including electronic health records systems.
Skill: Skill in the use of clinical software systems; skill in the use of standard office software; skill in keyboarding.
Ability: Ability to understand and effectively apply the concepts and practices of a trauma-informed philosophy in performing the work. Ability to work within strong professional ethics and rules of confidentiality to provide effective services to clients. Ability to effectively work with clients to encourage and facilitate engagement in the treatment process; ability to maintain a strong service orientation; ability to read and understand a service plan and to effectively coordinate identified services; ability to write and maintain timely and cogent notes, reports and other professional documents; ability to organize and execute work effectively under the pressure of time-sensitive deadlines; ability to work with grace and good humor under pressure and to meet multiple deadlines; ability to work cooperatively with other departments and service providers; ability to communicate effectively, both orally and in writing, and within trauma-informed care guidelines; ability to establish and maintain harmonious work relationships with co-workers at all levels of the organization, consultants, contractors, community partners, and consumers

- Provide service eligibility screening and coordination to assigned clients in accordance with Agency protocols and scope of position.
- Interview client to screen potential service eligibility.
- Process clients at intake and record initial client data including vital signs, current complaints, general health status, and specific client concerns. Provide unusual information to the provider for review.
- Deliver client communication relating to prescription renewals, current illnesses, chronic conditions, and health-related information and education.
- Provide basic client education and information about follow-up care at the conclusion of client’s visit.
- Facilitate booking of appointments and adjusting of appointment times; ensure, wherever possible, that services are provided in accordance with client preference; provide appointment reminder calls to clients; maintain regular in-person or telephone contact with clients to encourage engagement.
- With the prescriber, participate in client medication management appointments at various locations.
- Verify, identify and record all medications being used by clients.
- Process and log incoming medications.
- Maintain a sufficient inventory to ensure adequate supplies;
- And other duties as directed by the Prescriber as relates to medications.
- Performs tasks of “nursing” visits as directed by licensed medical provider.
- Assist the provider with treatments and procedures including preparation of client and equipment and post-treatment clean up.
- Operate bio-medical equipment following appropriate function and operating procedures. Report malfunctions immediately to supervisor or other personnel as appropriate.
- Provide direct client care in support of assigned medical providers.
- Record client data including vital signs, current complaints, general health status, and specific client concerns. Note unusual information for provider review.
- Assist provider during client exams as requested and required.
- Obtain information from outside agencies related to test orders.
- Document all pertinent information and actions in the client’s electronic health record.
- Actively participate in joint case planning and coordination with internal multi-disciplinary team, and agency psychiatrists.
- Prepare client files; ensure that files are complete including psychiatric and medical assessments and treatment service plans.
- Regular and reliable attendance; timely and professional communication; confidentiality.
- Participate in all team and agency meetings, as directed.

- Bachelor’s degree in a medical or social services-related discipline OR four (04) years’ experience as a medical assistant in a behavioral health or medical clinic setting.
- Graduation from an accredited medical assisting program.
- Current certification as a medical assistant within 6 months of hire.
- Experience working with individuals who have severe and persistent mental illness, substance abuse disorder and/or developmental disabilities is preferred.
- Bilingual Spanish/English is preferred.
- Must pass all criminal history check requirements as required by ORS 181.536-181.537 and in accordance with OAR 407-007-0200 to 407-007-0370.
- Must possess a valid Oregon or Washington Driver’s license, maintain an acceptable driving record, and be insurable for client service purposes and for travel between business offices, meetings and/or trainings.
