Southern Jersey Family Medical Centers, Inc. is looking for a Chronic Disease Care Coordinator to manage the care of our chronic disease patients.
Summary: The Chronic Disease Care Coordinator provides assistance to the physicians and nursing staff by coordinating care and services for patients in order to maximize use of available resources and to offer complete preventative care to those in need.
Hours: This is a full-time position that requires the ability to work a flexible schedule. General daytime hours Monday – Friday with at least one late night per week.
Position Responsibilities/Duties Include But Not Limited To:
• Assist in coordinating patient care for visits, ordered tests, and other healthcare needs
• Collaborate with providers and nursing staff to identify concerns that would prevent quality medical care
• Assist with patients enrolled in case management
• Provide effective and productive communication between patients and providers
• Educate and assist patients in the management, prevention, and maintenance of their health wellness state through the use of evidence based guidelines and best practices
• Assess and identify actual and potential patient problems by gathering relative data
• Address gaps in care of patients as identified in the electronic health record
• Work with physicians and nursing staff to develop plan of care for patients and make adjustments as needed
• Build and maintain relationships with both internal and external healthcare contacts and organizations in an effort to improve available patient resources
• Track patient outcomes and provide information to authorized personnel as requested
• Maintain electronic medical records (EMR) and other company information and ensure compliance with all State and Federal guidelines

Required Experience:
• 1 - 2 years of experience working within a primary or ambulatory care setting
• Case management experience preferred
• Excellent verbal and written communication skills and organizational skills
• Ability to educate, motivate and inspire
• Valid driver’s license
• Bi-Lingual preferred (Spanish or Portuguese)
• Computer skills
• High school diploma or equivalent
