SUMMARY:
The Care Coordination Supervisor, in addition to providing patient care duties of care navigator; is responsible for coordinating and supporting the service provision in the care management department and provides supervision to the care navigators and other staff as needed. The Care Coordination Supervisor participates in quality assurance activities and coordinates services for participants, and oversees day-to-day management and functioning of the sites and programs.
REQUIREMENTS:
- Provides orientation, training, supervision and support to assigned care navigation staff ensuring that assigned staff meet departmental documentation and service provision requirements. Participates in hiring and evaluation of assigned staff members.
- Create and facilitate ongoing training materials for new and senior Care Navigators in an effort to provide quality services to patients with the guidance of program associate and care management coordinator.
- Conduct administrative supervision with care navigators to provide supportive and ongoing feedback with the guidance of program supervisor
- Complete quality assurance activities under the guidance and supervision of care management coordinator
- Helps facilitate weekly team meetings and huddles; participates in in-house and external committees as requested.
- Assists with daily oversight of staff assignments and tasks to assure office and program functioning.
- Assists Directors with program reports, particularly as related to achievement of program performance targets and goals.
- Attends regional and statewide, and internal meetings and other relevant meetings as requested
- Demonstrates clinical competency and knowledge of intervention in areas such as, but not limited to: domestic violence; substance abuse; mental health; family systems; and, child abuse and neglect reporting laws and appropriate follow-up.
- Conducts community-based visits to patients and intensive community based outreach, as needed. Completes duties of care navigation role including completing evaluations, care plants, attending care team conferences, and track and follow-up with patient progress in the care management program.
QUALIFICATIONS:
- Bachelor’s Degree in social work, human services, psychology, nursing or related field preferred.
- Computer literacy with proficiency in MS Word, Excel, and PowerPoint.
- Demonstrated organizational, interpersonal, oral and written communication skills and the ability to handle multiple assignments at any time
- Ability to generate, interpret and analyze data from multiple sources
- Previous experience in role of care manager or similar job title preferred
- Previous supervisory experience (one year) preferred
Salary
50,000 - 60,000 /year
