Every day, Cambia's dedicated team of Care Advocates are living our mission to make health care easier and lives better. As a member of the clinical team, Care Advocates take an integrated approach to care management for members.
This role provides outreach as part of specific care gap initiatives or as an on-going part of a care management team. The advocates work with members to achieve their personal health outcomes by removing barriers and providing support and education. The Care Advocate team encourages engagement in health-related activities and programs and provides appropriate referrals to internal or external services and resources. All in service of making our members' health journeys easier.

What You Bring to Cambia:
Qualifications:
Equivalent combination of education and experience, preferred
Associate's or Bachelor's degree or clinical certification
Or Advisor or LPN
At least 2 years' experience providing direct patient care
Skills and Attributes:
* You'll need to possess strong communication and customer service skills, with the ability to effectively interact with individuals on sensitive topics, listen actively, and maintain confidentiality, while also having general organization, data entry, and administrative skills.
* You should be able to handle multiple tasks, prioritize your workload, and work independently to meet deadlines and achieve operational standards, with the ability to research and problem-solve creatively to reach a solution, and identify when to refer to others.
* You'll require basic knowledge of medical terminology, familiarity with health-related topics, and a general understanding of clinical services programs, such as Utilization Management, Care Management, and Disease Management, with at least 2 years of successful Care Advocate experience and ability to handle a wide range of member issues or interventions.
* You should be able to engage and motivate others to take action regarding their health, work collaboratively in a team environment, and contribute to team success, with the ability to understand clinical documentation and make appropriate care management referrals, and take on additional projects to develop your skills.
* You'll need to demonstrate strong communication and writing skills on sensitive and complex topics, with expertise in health-related topics, knowledge of clinical service department programs, and the ability to research, assess, and follow up on clinical issues within your role and scope of licensure, with direct patient care experience and intermediate computer skills, including experience with healthcare systems and documentation.
What You Will Do at Cambia:
* You'll be the first point of contact for members identified for our Care Management Programs, reaching out to them via phone or email to encourage engagement in health-related activities and provide education on preventive screenings, discharge and treatment plans.
* You'll work closely with Care Managers to ensure members' needs are met, making referrals to licensed clinical staff as needed, and utilizing our care management platform to record and track member interactions.
* You'll provide support, encouragement, and education to members in complex healthcare situations, such as hospital discharge or admission to a skilled nursing facility, to help them make informed decisions and navigate care options.
* You'll be responsible for managing a variety of cases that require general knowledge of clinical issues and healthcare practices, providing clinical assessments within your scope of licensure, and researching and providing accurate educational information for members.
* As a subject matter expert, you'll provide guidance and support to Care Advocates I and II, or individuals with less experience, assisting with training and trouble-shooting issues, and identifying areas for improvement in program processes and procedures.
