Breast Care Coordinator II - LPN - Women's Healthcare Associates, LLC

The Breast Care Coordinator II - LPN position navigates the patient's mammography results, ensuring the patients get appropriate follow up. They also assist the mammography department to ensure efficient patient flow by performing support duties related to imaging services.

*This is a Monday-Friday, 40 hrs/week position that will work at our NW Gynecology Center at Peterkort location.

DUTIES

• Proactively monitors mammography schedule and pool, communicates updates to patients and staff.
• Assures that patients are scheduled for the appropriate Breast Imaging service.
• Maintains Breast Imaging Coordinator Dashboard in EPIC with other clinical staff.
• Stocks procedure room as needed.
• Coordinates patient care and follow up imaging with internal and external medical staff.
• Performs the following functions within the EMR; accesses patient mammography report, reviews Gail score, documents mammography results, builds recalls, performs soft sign, routes mammography report to clinician's desktop.
• Requests external imaging and pathology reports for processing by HIM.
• Creates and links screening mammography orders to patient appointment.
• Answers telephone calls concerning general questions on breast imaging.
• Prepares and mails patient lay letters for all results including certified when indicated.
• Assists in maintaining cleanliness of mammography and ultrasound rooms as directed.
• Follows up on all patients to ensure patient's records are accurate with reports and images.
• Assists with scheduling and rescheduling screening, diagnostic and biopsy patients as needed.
• Places blocks in mammography scheduling templates in the EMR as directed.
• Assists with MQSA compliance and inspections and ACR accreditation applications.
• Understands BIRADS lexicon and routes mammogram results for indicated follow-up.
• Performs tasks that support Breast Imaging staff including nurses and Imaging Specialists.
• Assists and supports Breast Care Coordinator I
• Offers reassurance and support to patients pre and post procedure
• Maintains inventory of breast center supplies, including medications
• Assists in mammography suite and ultrasound room during procedures
• Prepares sterile tray for procedures and biopsies
• Assists with provider education and outreach

QUALIFICATIONS

• Licensed as an LPN in the state of Oregon.
• Current CPR/BLS certification required.
• Prior experience with EMR systems, Epic preferred.
