About the Role

In this role, you will play a vital role in optimizing patient care by providing comprehensive medication management services. You will collaborate with physicians, nurses, and other healthcare professionals to ensure the safe, effective, and cost-efficient use of medication.

Qualifications:

• Education:
  • Graduate of an education program approved by the credentialing body for the BCTXP certification.
• Work Experience:
  • Must have one of the following:
    • Successful completion of an ASHP-accredited PGY1 residency training program or equivalent experience (3 years per residency year). ASHP-accredited PGY2 or fellowship preferred.
    • Three years of clinical practice experience.
  • Meets HM standards of an ASHP-Qualified Residency Preceptor within one year of employment.
• Licenses and Certifications - Required:
  • PHRM - Texas Pharmacist License or eligible for licensure within 90 days.
  • PPC - Pharmacist Preceptor Certificate within 90 days.
  • ACLS (AHA) - For pharmacists assigned to Code Blue events (within 90 days).
  • PALS (AHA) - For pharmacists assigned to Code Blue events (within 90 days).
  • NRP (if applicable) - For pharmacists assigned to areas requiring neonatal resuscitation (within 90 days).
  • IV Certification (ACPE) - If applicable to primary work assignment (within 90 days).
  • BLS (AHA) - Within 90 days.
• Licenses and Certifications - Preferred:
  • BCTXP (Board of Pharmacy Specialties - Solid Organ Transplantation)
