Join our team as a Bilingual Case Manager on a contract basis, with the potential for permanent employment based on your attendance, performance, and our business needs. This role involves occasional travel and requires flexibility, including availability on Saturdays. The standard shift for this position is Monday through Friday from 8 am to 5 pm.
Responsibilities
As a Bilingual Case Manager, you will be responsible for managing a caseload of healthcare members with long-term care needs. This remote role involves monthly and quarterly member contact, with approximately 80% of your time dedicated to travel. Key responsibilities include:
Conducting member assessments and maintaining detailed notes.
Collaborating with members, caregivers, and providers to develop comprehensive care plans.
Monitoring service delivery and providing support through in-person visits and phone calls.
Coordinating referrals for additional services as needed.
Ensuring seamless coordination between providers and identifying areas for improvement in care plans.
Assisting in the development of informal or voluntary services to enhance member care plans.
Collaborating with discharge planners, physicians, and other stakeholders to ensure smooth transitions between acute and long-term care services.
Assisting members with complaints and appeals resolution.
Requirements
To qualify for this position, candidates should have:
A Bachelor's degree or a Registered Nurse License with at least 2 years of care management experience, or a Licensed Practical Nurse with at least 4 years of care management experience. Alternatively, candidates with 6+ years of care management experience will also be considered.
Experience in home health, discharge planning, or long-term care is preferred.
A valid driver's license is required.
