Must be Bilingual (English - Creole)

Job description:

• Maintain Quota of Home Care patients
• Open NEW accounts on a constant basis
• Upkeep of old and stable accounts
• Knowledge up to date home care regulations
• Collection of paperwork
• Communication with families and patients
• Use of CRM 
• Providing updates to their accounts
• Acting as a liaison between patient and Agency
• Working within agency structure 
• Field work
• Being able to assist with assessments
• Being a team player
