Job Summary:
The Providence Center Bilingual Behavioral Health Case Manager/Liaison (BHCM) will coordinate, initiate, as well as participate in Medical and Behavioral Health services for adults of Open Doors Health Center (ODH) who have co-occurring Serious Mental Illness (SMI) and physical health condition, or chronic disease, individuals with Substance Use Disorder (SUD) and individuals with co-occurring disorders in order to ensure a holistic, whole-person approach is taken towards providing awareness of community, educational and economic resources available to LGBTQ+ Community with a focus on individuals who identify as Hispanic/Latin X and other underserved populations.
DUTIES AND RESPONSIBILITIES: include the following. Other duties may be assigned.
SERVICE/ACCESS COORDINATION
BHCM will deliver brief, evidence-based practices shown to be effective for short term behavioral healthcare.
BHCM will deliver problem-solving therapy, behavioral activation, relapse prevention, safety planning and triaging to higher level of care in consultation with other care team members (PCP and Psychiatric Consultant).
BHCM will utilize short-term, 6-8 sessions for collaborative care practice.
BHCM will work with in collaboration with other healthcare providers on site an refer to these providers for specialized care (CBT for PSTD) or when greater than eight sessions of psychotherapy is indicated.
BHCM will be responsible for managing the patient registry and ensuring completion of all screening measures and individual care plans.
BHCM will attend weekly clinical team meetings to discuss clients/patients and use a stepped care approach for referring client/patients who would benefit from a higher level of care to other programs.
BHCM will be the system navigator and point of contact for shared ODH and TPC patients and families, with patients and families having direct access for asking questions and raising concerns.
Assist patients during their ODH medical appointment in understanding the relevant details of the visit, as requested by either the patient or the medical team. Provide interpreter services.
BHCM will assist with scheduling appointments for clients/patients with external medical specialists, as requested by the medical team and explain purpose of appointments in clear and simple language in patient’s spoken language when necessary.
BHCM will coordinate with ODH team and community services as needed to ensure that both current diseases, as well as preventative and health promotion needs are addressed as part of the holistic, whole-person, culturally-sensitive multidisciplinary team approach
TEACHING & EDUCATION
Teach and assist patients/clients to better manage their chronic diseases using realistic and SMART (specific, measurable, attainable, realistic and timely) goals.
Provide as-needed teaching to patients/clients during medical appointments in order to assist in the understanding of disease knowledge, medication compliance, specialty services recommended by primary care provider and/or any other medical needs the patient/client may require assistance in understanding.
Provide assistance with documentation such as Social Security, SSDI, SNAP, and when necessary Immigration documentation/application.
Assist patients/clients and their families in problem solving potential issues related to the health care system, financial or social barriers (e.g., transportation services or prescription assistance).
Use a step-by-step process when teaching skills.
Use positive reinforcement and encouragement.
Use a flexible approach with a diverse population.
Attend both mandatory in-service trainings and related conferences and provide feedback to staff on the content of educational programs attended.
Serve as a resource to staff regarding the LatinoX/Spanish community. Provide education and communicate on community resources and cultural matters. Interpret both language and cultural concerns for staff and other community service providers, as appropriate. Participate in in-service training and skill development activities within TPC and ODH.
ADVOCACY
Negotiate on behalf of the patient, as needed, with the medical and behavioral health treatment team, and patients themselves by creating reasonable goals, clarifying the issues to be negotiated, following through with the agreement reached, while remaining flexible and consider culturally-sensitive and holistic alternatives.
May assume advocate role on the patient's behalf to ensure approval of the necessary supplies/services for the patient in a timely fashion. Provide interpreter services as needed
RECORD-KEEPING
Complete quick notes for all care coordination services; progress notes daily.
Complete required interagency, Release of Information and other necessary paperwork.
Monitor confidentiality in accordance with ODH/TPC policy and legal requirements.
Follow required office procedures in a cooperative manner.
Initiate and process incident reports as per ODH/TPC guidelines. Complete all required TPC documentation in EPIC electronic record daily.
Adhere to Infection Control protocols.
Comprehensive knowledge of ODH Electronic Medical Record.
Transport shared ODH/TPC clients as needed

Requirements:
Bachelor's degree in Human Services required.
Experience in cross-cultural education, training, and consultation required.
Experience working with diverse populations, specifically LGBTQ+ Hispanic/Latin and other underserved populations.
Experience with Rhode Island substance abuse system and its network agencies. Awareness of the general trends in confronting the problems of substance abuse.
Interest in working in a primary care clinic with medical staff strongly encouraged.
Bilingual fluency in English and Spanish (reading/writing/speaking).
Must have valid driver's license and insured auto.
