Position Overview
The Behavioral Health Specialist will work closely with the Social Work team, providing behavioral health services and advocacy for participants under the Program of All-Inclusive Care for the Elderly (PACE) model. This role is essential in delivering high-quality behavioral health interventions, including individual and group therapy, assessments, and care planning for senior participants.
Key Responsibilities
- Behavioral Health Assessment & Care Planning: Conduct behavioral health assessments to identify needs, preferences, and goals, participating in interdisciplinary team meetings to establish personalized care plans.
- Therapeutic Intervention Delivery: Document and deliver interventions including symptom management, crisis intervention, counseling, and care management.
- Therapy Facilitation: Lead individual and group therapies through diverse formats, such as art therapy, music therapy, psychosocial drama, and counseling to promote participants’ social engagement.
- Training & Education: Provide ongoing training to staff on behavioral health symptoms and intervention techniques.
Required Qualifications
- Education: Master’s Degree in Behavioral Health (MSW) or Doctorate in Psychology (PhD) required.
- Licensure: Licensed by the California Board of Behavioral Sciences as an LCSW or by the California Board of Psychologists as a Psychologist.
- Experience: Minimum of five years in behavioral health, with at least two years focused on therapy for a frail or elderly population.
Skills & Certifications
- LCSW or Licensed Psychologist: Essential certification for practicing in the role.
- Therapeutic Proficiency: Strong skills in behavioral health assessment, therapy facilitation, and care management.
- Interpersonal Skills: Effective communication and empathy for delivering patient-centered care.
