The Behavioral Health Counselor/Case Manager will provide support and guidance to individuals facing mental health challenges. This role involves assessing client needs, developing treatment plans, and coordinating services to promote overall well-being.Responsibilities:

Conduct assessments and develop individualized treatment plans

Provide counseling and support to clients and their families

Coordinate with community resources and services

Monitor client progress and adjust plans as needed

Maintain accurate documentation and case records

Requirements:

Bachelor's degree in psychology, social work, or related field

Strong communication and interpersonal skills

Knowledge of mental health resources in the community

Experience in case management preferred
