Associate Manager of RRH - Illumination Foundation

Homelessness is the largest social and public health crisis in California. Illumination Foundation (IF) is a growing non-profit organization dedicated towards disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services and children's and family programs. IF currently has 13 facilities with 22 micro-communities scattered across Orange County, Los Angeles County and the Inland Empire.

Job Description

The Associate Manager of RRH will work closely with the Program Manager/Director to ensure contract compliance and outcomes are met. Additionally, they will help coordinate program tasks, monitor program data, and other tasks deemed important to the implementation of the programs. The Associate Manager of RRH will ensure intakes are completed following grant and contract requirements.

The salary range for this position is $28.00-$31.00 per hour, depending on experience. 

Responsibilities

• Facilitate weekly case conference meetings. 
• Model trauma-informed, harm reduction, and client-centered care in all client interactions. 
• Address client grievances as necessary and provide support to staff in addressing client conflicts. 
• Provide guidance to staff as it pertains to providing care to clients. 
• Implement quality assurance processes to ensure effectiveness and efficiency of program operations and compliance efforts. 
• Work with the manager to develop and implement policies and procedures to ensure compliance with program guidelines and objectives. 
• Conduct regular audits and monitoring activities to assess compliance with program requirements, identify areas of non-compliance, and implement corrective actions as needed. 
• Provide training to staff on compliance requirements, including data privacy regulations, and programmatic guidelines. 
• Oversee the collection, analysis, and reporting of data related to program compliance, ensuring accuracy and integrity of data systems. Assist with auditing client files as deemed necessary 
• Ensure case managers enter client documentation such as case notes, client records, vital documents, insurance cards, SSI award letters and other essential documents in internal and external databases in a timely manner. Evaluate completeness and quality of case notes that are entered. 
• Document any other such desirable reporting metrics as outlined and required by contracting entities. 
• Prepare and submit reports to funders and other stakeholders as required, documenting program activities and compliance efforts. 
• Communicate concerns with the CM team to the manager. 
• Ensure clients have active healthcare insurance enrollments. 
• Provide technical assistance to case managers to refer clients to IFMG. 
• Submit referrals to CalAim programs (ECM, Community Supports, etc.) using the CalOptima Connect portal. Monitor CalAim referrals for our housing clients to ensure connection to services. 
• Maintain, and organize client case files with the necessary documentation. 
• Attend regular individual supervision session with immediate supervisor to discuss program, organization, client’s progress, new strategies to meet client’s level of need and improve treatment plan 
• Ensure referred clients meet program eligibility and intakes are completed promptly. 
• Complete monthly check requests for rent payments for the Rapid Rehousing Program. 
• Network with community partners.
• Knowledge and applied practice of HIPAA parameters

• Adept levels of awareness and utilization of Illumination Foundation’s EMR system 
• Professional and discreet interpersonal engagement with all established and prospective clients as well as fellow care team members

• Sufficient knowledge and utilization of peripheral services which may be necessary to fully address client needs, as well as the community support services being offered in the particular county of operation 
• Continued sensitivity to and awareness of cultural and linguistic diversity 
• Honesty, transparency, and fluidity in all client engagements and engagement attempts 
• Uphold high ethical standards in all aspects of program compliance and operations, ensuring transparency, fairness, and accountability. 
• Awareness of Illumination Foundation models of care such as harm reduction, and social determinates of health  Astute knowledge of, and diligent effort being made on assembling and following up on comprehensive assessments and care management plans

Preferred Experience/Minimum Qualifications

Required:

• Be flexible, open, conscientious, and have a good / positive attitude
• Possess marked social aptitude / a keen ability to work in social environments
• Excellent communicator and understand flexibility and variability in different communicative styles.
• Bachelor's degree in social services and 2 years' experience in the Social Services field.
• Must also have a valid CA driver's license and a good driving record 
• Strong computer skills including the ability to prepare correspondence and reports in Microsoft Word, and summarize and report data in Excel spreadsheets.

Preferred:

• Bilingual in Spanish and English is helpful, highly desirable
