Job Summary:The incumbent will oversee programmatic operations for the BAHN Lead Health Home’s Care Management Agency. Working with the AVP of Operations to provide oversight and ensure quality service delivery is provided to members of the program.
Responsibilities:- Provide direct supervision to General Manager(s) and closely monitor and manage the daily operations of the health home program, ensuring that all services are delivered effectively and efficiently.
- Hiring, training, and evaluating team members.
- Oversee the development and implementation of care plans for patients, ensuring that they receive comprehensive, coordinated care.
- Develop and implement programs and initiatives to improve patient outcomes, service delivery, and overall program effectiveness.
- Ensure that the health home operations comply with federal, state, and local regulations, as well as organizational policies and procedures.
- Implement and manage quality improvement initiatives to enhance service delivery and patient satisfaction.
- Collect and analyze operational data to identify trends and areas for improvement, and prepare reports for senior management and stakeholders.
- Collaborate with internal and external stakeholders, including healthcare providers, community organizations, and regulatory bodies, to enhance program services and outcomes.
- Participate in strategic planning activities to align the health home program with broader organizational goals and objectives.
- Serve as an advocate for patients, ensuring they receive the necessary support and resources for their health and well-being
- Leverage health information technology, such as electronic health records (EHR), to improve patient care and operational efficiency.
- Perform other duties as assigned by the Director of Operations.
Education and Experience:
- Bachelor's degree in social science, healthcare administration, social work or human services; related experience in health care, specialized populations, and HIV treatment and prevention. Master's degree in social work with NYS license is preferred.
- Three (3) years’ experience working with currently or formerly homeless populations and individuals with chronic conditions, including HIV, mental health disorders, and those living with HIV in a social service or medical setting; with a strong background in staff supervision experience.
- At least (2) years in a supervisory position in a health care or social service setting.
- Demonstrated knowledge of Care Management and Health Home outreach & enrollment
- Strong knowledge of Microsoft Office Suite (Word, Visio, Excel).
