Education and Experience - Case management: 1 year (Preferred)
- Senior care: 1 year (Required)
Work Hours - Monday to Friday, 9:00 AM to 5:30 PM; days may be flexible based on the need
Duties and Responsibilities CARE COORDINATION - Coordinate with various stakeholders to ensure client care plan is accurate, up to date, and meeting the needs of the client
- Coordinate and attend client care conferences
CLIENT SATISFACTION & RETENTION - Receive caregiver and point of contact reports and requests and make sure they are documented and acted on
- Follow up with client and their point of contact to make sure any concerns have been addressed in a timely manner • Communicate with client and point of contact on a regular basis regarding the care plan and client or family needs
- Build and nurture relationships with clients and their point of contact through regular contact, communication, and in-person visits
- Provide quality assurance visits and check-ins DATA MANAGEMENT
- Monitor re-assessment deadlines
- Document client interactions and pertinent caregiver reports
- Monitor assigned accounts
- Other duties as required
