SUMMARY
Housing for Health (HFH) is a program office within Health Services Administration, a division under the Los Angeles County Department of Health Services (DHS). HFH was created and put into implementation in support of the Los Angeles County Homeless Initiative recommendations in response to and in support of the County’s effort to address and combat homelessness in the communities residing within Los Angeles County. Our organization follows a hybrid work structure where employees work both remotely and from the office, as needed.
HFH’s Housing and Services team works in partnership with contracted agencies to permanently house people and provide non-time limited services in housing. As such, HFH is a key player and supports many of the Los Angeles County Homeless Initiative recommendations in response to and in support of the County’s effort to address and combat homelessness in the communities residing within Los Angeles County. The RAD (Referral, Access, and Data) within CHAMP, HMIS, and RMS systems to ensure the tracking and assistance of all new referrals into the Housing For Health Program. This team is looking for applicants with strong data analytical skills and knowledge of Los Angeles homeless services and case management. RAD ASA position will be tasked with coordination and operation of Intensive Case Management Services including but not limited to referral, technical assistance, program management and evaluation in coordination with our contracted agencies.
ESSENTIAL FUNCTIONS
Include but are not limited to:
- Assists in developing policies and procedures, setting programmatic benchmarks, and collecting and analyzing outcomes data and quality of data for ICMS participants.
- Provides regular programmatic updates to oversight committees, other participating departments, partner agencies, contracted service providers, and stakeholder groups.
- Assists in the design and implementation and day to day operations of projects, and prepare project reports including, service utilization, performance data, and program outcomes.
- Conducts contract monitoring activities including annual administrative and programmatic monitoring.
- Assists in coordination and collaboration with all project partners (e.g., participating departments, partner agencies, contracted service providers, stakeholder groups) to ensure all elements of the projects operate in the most efficient and effective manner.
- Provides technical assistance and training to project partners and develop, recommend, and implement program modifications.
- Assists in evaluating the overall success of assigned projects and reports findings to project partners and stakeholders.
- Monitors data quality and implement data improvement procedures as needed.
- Assists with requests for approvals for tenancy support services.
- Other duties as assigned.
JOB QUALIFICATIONS
Minimum Qualifications
3 or more years of highly complex administrative experience in the health, housing, social services, or related sectors
Desirable Qualifications
- A Bachelor’s degree in Social Work, Psychology, Human Services or in a related field preferred.
- Ability to pay close attention to detail.
- Good communication skills with colleagues, external partners, and contractors.
- Ability to conduct trainings and facilitate meetings.
- Ability to work in a professional manner, with courtesy and tact, and maintain confidentiality and security of documents and information
- Ability to work collaboratively with multidisciplinary professionals and partners both internally and externally
- Must possess strong organizational skills initiative and follow-through.
- Computer literate and skilled in use of MS Windows-based programs, including MS Office (Word, Excel, PowerPoint)
- Knowledgeable of data systems utilized across DHS and the homeless system of care, e.g., CHAMP, ORCHID, Clarity, Resource Management System, etc.
Certificates/Licenses/Clearances
- A valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.
- Successful clearing through the Live Scan and the Health Clearance process with the County of Los Angeles.
