Assistant Director of Social Work - Briar Place Nursing and Rehabilitation

Job Description:

Communicate with interdisciplinary teams regarding care of residents

Manage caseload and behaviors

Conduct meeting

Conduct bi-weekly audits on Advanced Directive for all residents.

Conduct weekly audits on Group notes for appropriate documentation follow up.

Ensure all follow up from meetings are completed by end of shift.

Assist with training and in-servicing of staff.

Completing corrective actions

Conduct audit on all new admissions and send out follow up to be completed.

Monitor MDS completion and email schedule weekly.

Audit monthly stats and send out appropriate follow up for residents who fall below 50 percent for month.

Audit monthly notes for accuracy and correct data. Ensure I.O. data is incorporated in documentation.

Audit assessments, care plans, and progress notes for all MDS assessment.

Monitoring dining ensuring all trays are passed during meal times.

Communicate all changes in resident condition, functioning, and behavior to PRSD.

Update concern binder weekly and have administrator sign off on all concerns and files them.

Knowledge of HIPPA, Department of Aging, and Sub Part S.

Facilitate and advocate and for all residents.

Knowledge of abuse and neglect and protocols for reporting incidents.

Any other job duties as assigned by department managers.
