Job Summary
The primary purpose is to assist the DON in planning, organizing, and directing the day-to-day functions of the Nursing department to ensure that the highest degree of quality care is maintained at all times, as directed by the Administrator or the Director of Nursing. Reports to DON.

Job Duties & Responsibilities
· Will act as DON in absence of DON.
· Assist the DON in planning, developing, organizing, implementing the day-to-day functions of the nursing department in accordance with all governing rules.
· Assist in nursing education.
1. Provide in-service training in the following areas for all CNAs on a monthly basis.
a. Catheter Care
b. Pericare
c. Infection Control
d. Oral care & hygiene
- Oxygen Application
- Safe transfers
2. Return demonstrations for CNAs.
- Pericare for all CNAs upon orientation, quarterly, and as needed.
- Catheter care for all CNAs upon orientation, quarterly, and as needed.
- Hand washing for all CNAs upon orientation, quarterly, and as needed.
- Provide in-service training in the following areas for all Nurses on a Quarterly basis.
- Infection Control
- Oral Care and hygiene
- Oxygen Administration
- Medication Pass and proper documentation
- Mantoux Testing
- General nursing issues
- Return Demonstrations for Nurses
- Mantoux testing- semi annually
- Medication Pass-semi annually
· Participate in developing, maintaining, and updating our written policies and procedures.
· Ensure that reference material maintained at nursing station is current.
· Make written and oral reports/recommendations to the DON as necessary concerning the operation of the department.
· Ensure that all nursing personnel are following their respective job descriptions.
· Participate in the development, maintenance, and implementation of the facility’s QA program for the nursing department.
· Assist the resident and Social Services in planning the nursing services portion of the resident’s discharge plan.
· Documentation
- Chart Audits
- Admission audit within 72-hours- follow 72-hour protocol.
- Audit of all charts on each respective floor Quarterly
- Lab reports; analyze for your floor all labs for
- Physician notification
- Nurse noted lab and new orders.
- New orders are followed through.
- PT/INR- in absence of evening supervisor, log all daily reports from lab and ensure that proper action is being taken.
- Other Documentation Monitoring
- Infection/ Isolation
- Change of condition
- Review nursing notes to ensure that they are informative and descriptive,
- Incidents and Accidents
- Play integral part in prevention with QA rounds on your floor.
- To complete investigations and conclusions of incidents and accidents on your floor
- Assist DON to update incident and accident investigation and prevention policies and procedures.
- Notify DON of all reportable incidents or accidents prior to reporting to the state.
- Monitor that nursing documentation is monitored per protocol.
· Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc, as necessary.
· Serve and participate in and attend various committees of the facility (i.e., Infection control, QA, Safety Committee, etc.)
· Attend and participate in Care Plan meetings as necessary.
· Participate in the weekend administrative duty schedule.
· Participate in the supervisory on-call schedule.
· Assist in determining and maintaining staffing needs in the nursing department.
· Rotate on call pager for staffing concerns.
· Assist in the Recruitment and selection of nursing personnel.
· Ensure that a sufficient number of nurses and CNAs are available for each shift.
· Oversee scheduling work assignments and duty hours with co-ADON and Staffing coordinator.
· Assist in preparing performance evaluations.
· Make daily rounds to ensure that all nursing personnel are performing their work assignments.
1. At least twice per day on your floor looking for the following.
a. Proper catheter bag placement
b. Residents groomed well.
c. Oral hygiene
- Proper positioning
- CNAs using gait belts.
- Staff wearing name badge.
- Staff interacting with residents, families, and other staff appropriately.
- Oxygen tubing off floors
- Humidifiers dated.
- Tube feedings are documented with, date, rate, product, and time.
- Proper footwear
- Call lights within reach
- Monitor nursing care to ensure that all residents are treated fairly, and with kindness, dignity, and respect.
- Ensure that all nursing care is provided in privacy and that personnel knock before entering a resident’s room.
- Signs of change in condition
i. Dehydration
ii. Lethargy
iii. Mental status change
iv. Overall status
- Monitor Staffing
- Check for call offs.
- Ensure staffing needs are met.
- Daily problem solving and review with staffing coordinator.
- Participate in rotation to work the floor for staffing needs as necessary.
- Dining Rooms
- Monitor meal service daily.
- Ensure that all staff assigned to the dining room is in the dining room in a timely manner.
- While staff are feeding, they should be:
i. Seated
ii. Speaking with the resident not around them to others
iii. Staff should not shovel food.
iv. Ensure that staff is offering substitutes.
v. Ensure that CNAs are notifying nurses when residents do not eat.
- Medication Pass
- Completed in a timely and appropriate manner.
- Carts in hall and locked
- Out of date medications
- Dated insulin in med room.
- Apple sauce dated.
- Ensure that narcotics are signed out.
- QA for glucometers
- Monitor and report any medication errors to DON.
· Monitor absenteeism, tardiness, and overtime. (Be creative to resolve these issues)
· Ensure that departmental disciplinary action is administered fairly.
· Participate in selection of residents for admission.
· Coordinate with Admissions Director for special needs (i.e. oxygen, special mattresses.
· Greet new admissions upon arrival.
· Make rounds with doctors when necessary.
· Ensure that residents who are unable to call for help are checked frequently.
· Attend and participate in continuing education programs designed to keep you abreast of changes in your profession as well as to maintain your license on a current status.
· Monitor nursing personnel to ensure that they are following established safety regulations in use of equipment and supplies.
· Ensure that nursing work areas are maintained in a clean and safe manner.
· Ensure that all nursing personnel follow established departmental policies and procedures, including appropriate dress code.
· Ensure that an adequate stock level of medications, equipment, etc., is maintained on site to meet the needs of the residents.
· Maintain confidentiality of all resident care information.
