Job Overview
Provides clerical and administrative support to the Care Management department staff to facilitate hospital discharge plans and other activities, as needed. Provides clear communication across the continuum to improve patient outcomes. Serves as a liaison between the hospital and external vendors facilitating optimal communication, care coordination and discharge planning. Coordinates the necessary documents to secure authorization for post-acute services and demonstrates knowledge of the Medicare, Medicaid and Commercial payer discharge process.
Qualifications:High school diploma or equivalent with previous work related experience plus three to six months on the job and orientation; Associates degree preferred. Excellent writing, interpersonal and organizational skills required. Ability to exercise judgement within defined programs and practices to determine appropriate action. Strong computer skills and familiarity with Windows based programs
