Assessment Specialist - GA: Dodge and Surrounding Counties

CareStar, Inc. is currently seeking an Assessment Specialist in the Clinical departmentto identify, coordinate and oversee the delivery of services to enhance the health, safety and general well-being of participants.

THE POSITION

CareStar is looking for motivated individuals who:

• Possess a Bachelor's degree in social work, sociology, psychology, human services, or a related field, or
• Registered, professional nurse currently licensed to practice in the State of Georgia AND two years' experience in the human services or health-related field, or
• Possess an associate degree in human services or health-related field AND a minimum of two years' experience in working directly with aging or disability populations.
• Have specialized knowledge of older person, the aging process, and long-term care issues, and person with disabilities and know disability etiquette.
• Effective organizational and communication skills
• Willingness and ability to work flexible schedule to meet needs of families including reliable transportation, valid driver's license and car insurance as required by state law, and the ability to travel.
• Adheres to the CareStar Rule in performance of job responsibilities.
• Understands and complies with CareStar Policies and Procedures.
• Maintains confidentiality as related to patient information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offense.
• Follows the Acceptable Use Policy while using any information systems owned or controlled by CareStar, Inc.

This position is open in Georgia and is responsible for Dodge and surrounding counties
