General Summary:
The Annual Wellness Visit Nurse collaborates with primary care providers and health teams to deliver direct patient care, focusing on identifying patients for Annual Wellness Visits and completing them efficiently. Responsibilities include following protocols, educating patients on health prevention, and documenting care accurately.

Responsibilities:

Follow AWV protocols and complete preventive services.

Document eligibility and quality gaps.

Collaborate with clinical teams for efficient AWV completion.

Prepare injections and maintain patient records.

Ensure HIPAA compliance and report findings to providers.

Required Education and Experience:
High School Diploma or GED; Licensed Practical Nurse with 2+ years of clinical experience, including 1 year in an outpatient setting. Preferred experience with Medicare Annual Wellness Visits.
