Our growing Population Health department is looking for a Nurse Practitioner! The Population Health Nurse Practitioner plays a key role in providing the highest quality of patient care while the patient is at Union Health. Our Population Health team supports overall patient wellness and transitions of care. The Population Health Nurse Practitioner will work closely with our post-acute care RN navigator for optimal patient management.

Population Health Nurse Practitioner job details:
Position Details:
- Full Time/80 hours per pay, no weekends/no holidays/no call
- Monday-Friday 8a-4:30p
- This APP will primarily see and follow patients in skilled nursing facilities for wellness, readmission prevention, and to support transitions of care.
- May have some time in the office setting as well
Essential Job Duties:
- Perform history, physical exams, and preventive health measures under physician collaboration.
- Collaborate with SNF and ACO Medical Directors to enhance coordinated care and advance directive recommendations.
- Prepare formal care plans and length of stay estimates for patients as necessary.
- Assist in completing patient Advanced Directives and educate nursing home staff on patient wishes.
- Coordinate with healthcare professionals, manage at-risk patients, reduce hospital readmissions, and ensure medication reconciliation.
Qualifications:
- Graduate from an accredited MSN program with certification from ANCC or AANP or obtain certification upon hire
- Must have Indiana RN license
- Must have active APRN/CSR license, or be eligible for licenses
License/Certification:
- BLS certification
