About the Job
This Advanced Practice Provider will join our Senior Medicine Team at Cottonwood Clinic in Murray, UT. As part of the team, you will support and be supported by a lead geriatrician, other APPs, MAs, Nursing, and administrative staff.
Once fully trained and oriented, this APP will quickly build their patient panel. Due to the high patient volumes in this clinic, to be successful in this position, at least two years' experience working as an APP in Adult Medicine is required. We will not be considering new grads or any APP who does not have previous Adult Medicine experience.
As a Geriatric Medicine APP, you will provide care to patients 75 years of age and older. Many of these patients will have multiple chronic conditions, requiring frequent office visits to manage.
In this field you will develop long term relationships with your patients as you assist them on their journey for a healthy life. Establishing these long-term relationships is rewarding and you often treat multiple family members and learn how to care for those in your community. The emphasis is on keeping people well, not just treating a disease state. You will focus on value-based care initiatives and will be the primary resource for your patients regarding their overall physical and mental health.
Some of the chronic conditions you will manage are: Diabetes, Dementia, Hypertension, Congestive Heart Failure, Chronic Kidney Disease, COPD/Asthma, Depression/Anxiety, Sleep Apnea, GERD/IBS, Stroke/TIA, Anemia, along with preventive care.
Cottonwood Clinic is open Monday - Friday 8am to 5pm. This APP will typically work weekdays and share after-hours call every couple weeks.
This position is full time salaried and fully benefits-eligible, which includes generous continuing medical education funds to support you in your growth as an APP.
Job Essentials
Position Specific Duties:
This position is accountable for the provision of Adult-Geriatric patient care and other duties as permitted by licensure and defined within Credentialing and Privileging documentation (hospital), Credentialing and Scope of Practice (Medical Group), Consultation and Referral Plan (NP), Delegation of Services Agreement (PA) (DOPL) or other similar documentation as appropriate.
Clinical Practice:
Perform Adult-Geriatric patient assessment to obtain a complete and accurate record of the patient's clinical information, medical symptoms, and support system issues. Order tests and exams to obtain additional information needed to ensure proper and complete care of patients with chronic illness.
Based on assessment, diagnose patient needs and determine a plan of care, patient goals of care, and palliative Care needs. Interpret patient care tests and exams to obtain needed information to guide care.
Provide for the treatment of the patient (and oversee the treatment provided by others) including appropriate prescriptive practice to achieve optimal clinical outcomes for the patient. Utilize advanced specialty knowledge to ensure that the care provided is appropriate, effective, timely, and cost efficient, matching the patients goal of care.
Perform diagnostic and/or therapeutic procedures as defined by Scope of Practice.
Appropriate and timely documentation of clinical care. Code and bill accurately per established protocol and procedure. Ensure that records are detailed and complete, and comply with related standards and regulations.
Education:
Conduct and/or coordinate Geri-centric education to increase the knowledge and health literacy of patient, patient family, and others. Providing appropriate instruction based on patient needs to support the continuum of care.
Train other staff in areas of clinical expertise. Provides teaching of students as appropriate to increase their clinical knowledge and ensure safe patient care.
Stay abreast of current trends in healthcare practice and participate in continuing medical education to ensure optimal clinical knowledge and effective patient care skill.
Consultation/Collaboration:
Coordinates and collaborates patient care efforts with other care and community providers/partners.
Communicates clinical information timely, accurately, thoroughly, and appropriately to other providers of patient care.
May collaborate with other care providers in the effective sharing of practice expertise.
Research:
May participate in research and utilizes evidence-based practice to promote quality patient care, protocol development, care process model development, and best practice standards for assigned patient population.
Supports professional development through research processes.
Minimum Qualifications
Current state license as an Advance Practice Registered Nurse (APRN).

Certification as a Nurse Practitioner (Credentialing requirement).

Eligible for State Controlled Substance License II through V.
- and -
Eligible for Federal DEA Certificate II through V.
- and -
Basic Life Support Certification (BLS) for Healthcare Providers.
- and -
Other certification as required (e.g. ACLS, PALS, TNCC, NRP).
Preferred Qualifications
Masters degree or higher.
Applicable certification from a recognized certifying organization such as American Nursing Credentialing Center (ANCC) or the American Academy of Nurse Practitioners.
Applicable experience as defined by position leadership.
