Position: Advanced Practice Provider (FNP or PA)
Location: On-site, flex between locations as needed.
Reports to: Chief Medical Officer
Type: Full-time
Charlotte Community Health Clinic offers high-quality medical, dental and behavioral health services for children and adults. We are currently seeking a Physician Assistant or Family Nurse Practitioner to join our team. Responsibilities include providing high quality medical care, patient education, strict adherence to HIPAA guidelines, enjoy working in team environment, and involvement in Quality Improvement activities. This position will report to the Chief Medical Officer.
Duties:
- Provide medical services to patients, inclusive of all functions necessary to provide quality medical care
- Develop treatment plans for acute and chronic diseases
- Educate and guide patients on disease prevention, disease management and healthy lifestyle habits
- Provide monitoring and continuity of care between patient visits
- Contribute to teamwork and communications.
- Work within a clinical care team to achieve patient and team goals, participate in regular staff meetings, and collaborate with other disciplines to achieve desired outcomes
- Work within the provider team, including participating in regular staff meetings and collaborating with other providers to achieve desired outcomes for the clinic’s patient population as a whole
- Collaborate with physician clinical supervisor for chart review and continued education
- Provide administrative, staff development and student supervisory responsibilities as needed
- Participate in Quality Improvement activities
Required Skills and Values:
- 2 years of APP experience is required.

Qualifications:
- Master's degree in Physician Assistant studies or Nurse Practitioner studies
- Successful passage of the Physician Assistant National Certifying Exam or Nurse Practioner exam.
- If a NP, FNP board certification is required.
- Current unrestricted PA or NP licensure in North Carolina
- Current DEA licensure.
- Strongly preferred: 2-4 years of experience in community-based primary care, including work with low-income, uninsured patients.
- Bilingual in English and Spanish is strongly preferred.
- Experience with EPIC EMR preferred.
