This continuum is designed to meet the complex needs of each individual and creates a continuum of services that are able to meet the needs of children, adults, and families through a continuum of services utilizing an integrated care model. We are dedicated to seeking new innovative solutions that help us provide needed services more efficiently while maintaining stable, sustainable growth.
Title: Advanced Practice Nurse (APN)
Salary: Starting at $180000 / annually depending upon education, experience, and licensure.
Responsibilities:
- Complete medication audit (weekly)
- Provide consultation, as needed
- Assess the physical condition of the youth under the direction of the Psychiatrist and integrate findings into the youth's treatment plan
- Provide education and support to direct care milieu staff on the administering of medications and possible side effects, under the direction of the Psychiatrist
- Monitor medication
- Provide medication education to youth and family
- Attend treatment team meetings
- 24/7 availability as needed
- Current New Jersey advance practice nursing license
- One-year direct care nursing experience with children
Assertive Community Treatment Services for Youth (Youth ACT)
The program operates throughout Cape May and Atlantic County and delivers a multidisciplinary mobile team to provide intensive and immediate access to treatment services in the community. The program works with youth ages 5-20 who present with significant mental health needs, challenging behaviors, and/or are at risk of psychiatric hospitalization or out of home care.
