Admissions / MDS Coordinator (RN) | Full-Time. - Advanced Therapy Center at Chesterwood Village.

We are seeking an Admissions/MDS Coordinator to join our team in West Chester, OH. This full-time position offers a pay range of $36 to $43 per hour for Registered Nurses, with additional shift differentials for evenings and weekends.

Responsibilities include:

• Overseeing the MDS process and collaborating with MDS nurses for assessments.
• Managing patient admissions, MDS assessments, and care planning.
• Completing required submissions and ensuring error-free documentation.
• Developing and updating comprehensive care plans.
• Monitoring resident needs and changes in status.

Qualifications:

• Current RN license in Ohio.
• Flexible schedule and ability to work independently.
• Understanding of the MDS process and knowledge in PDPM & RUG.
