Position Summary:
This is a position responsible for the admission and delivery of care to incoming patients.
Duties include: Processes admissions and discharges to maintain a full census; obtains required orders and assessments. Provides health education, including medication information, to patients & their families as needed. Provides notification of incidents, medical concerns, medication changes to families & referral sources. Participates in PI and committees as directed.
Directs and manages the referral process for admissions into the facility. Works closely with the Director of Admissions, Director of Clinical Services, Medical Director and Director of Nursing to ensure acceptance of patients is in keeping with the admission & exclusionary criteria of the hospital & are appropriate for admission. Gathers required information from referral sources to make a decision about the appropriateness of the patient for admission; maintains a referral log that identifies all referrals, dates, person making the referral, and outcome. Provides written summary of each patient being admitted that includes identifying information, behaviors, reason for referral or need for treatment & background. Assumes responsibility for generating referral packet and completing all relevant/required paperwork. Conducts initial certification assessments. Conducts managed care pre-certifications and completes all required paperwork to ensure prompt processing of insurance claims.
Minimum Qualifications:
Education: LPN licence required.
Experience: Minimum of one (1) year of recent experience; psychiatric preferred. ER experience or geriatric care may substitute.
Age-Specific Competencies: Adults and Adolescents
Licensure/Certification: CPR and CPI Certification
HIPAA: Has access to and adds information to the Medical Record. May only access medical records of assigned patients.
Work Demands/Environment:
Requires full range of body motion, including handling and lifting patients, manual and finger dexterity and eye-hand coordination. Requires standing and walking for extensive periods of time. In addition to lifting of patients, occasionally lifts and carries items weighing up to 50 pounds. Requires corrected vision and hearing to normal range. Requires working under stressful conditions and/or working irregular hours. Potential exposure to communicable diseases, toxic substances, body fluids and other conditions common to a hospital environment.
