Admissions Director - Mayfield Heights Nursing and Rehabilitation

Mayfield Heights Nursing and Rehabilitation is a 150-bed facility providing long-term care, skilled rehabilitation, intensive outpatient care, and memory care. 

We are currently seeking a full-time Admissions Director to join our team. The ideal candidate must be devoted and passionate about their chosen career. Leadership abilities and effective communication skills are necessary for this position.

We are located at 6757 Mayfield Rd., Mayfield Hts., OH

Responsibilities:

• Establish and maintain positive relationships with referral sources to promote skilled services
• Coordinate the referral process from the identification of the appropriate payer source to the analysis of patient information
• Responsible for achieving the proposed company census and revenue objectives
• Collaborate with patient or representative, physicians, residents, nursing staff members, rehabilitation therapy staff members, and social work staff members to ensure seamless transfer to the facility
• Communicates information effectively to physicians, supervisors, facility administrators, and any other pertinent personnel in a timely manner
• Analyze patient needs and match with facility services to plan for an appropriate facility placement
• Other duties as assigned by supervisor

Qualifications:

• Prior experience in a long-term care setting preferred
• Licensed Practical Nurse (LPN) or Registered Nurse (RN) qualifications optional
• Minimum of two (2) years experience in long-term care
• Proven ability to successfully market the services of the organization.
