Admissions Coordinator Job Summary:
This position functions as a clinical liaison between La Rabida Children's Hospital and other hospitals and healthcare organizations that serve as a referral base for La Rabida. The Admissions Coordinator regularly works with Case Managers, Physicians, and Families to assure that appropriate patients are referred, prior authorization is approved and patients are admitted to La Rabida in an appropriate time frame. This position also functions as the key clinical position in relationship building and maintaining and establishing new referral patterns.
Essential Admissions Coordinator Functions:
- Accepts and responds to all inquiry calls regarding possible referrals within one business day (for planned admissions) and immediately for all emergency and unplanned referrals. Facilitate communication between the referral source and appropriate clinical staff at La Rabida. In conjunction with nursing, respiratory, and the providers, facilitates the timely admission and/or transfer of patients to La Rabida. Acts as patient advocate in arranging for transfers. Chairs weekly La Rabida Admission meeting. 40%
- Provides consultation to referral sources regarding the appropriateness of patients for admission to La Rabida. Handles complex discharge planning issues and provides up-to-date information regarding La Rabida services and programs. Attends weekly rounds at referring hospitals and other agencies as appropriate. 20%
- Performs site visits for prescreening assessments as determined by department protocol. Gathers necessary information to determine the appropriateness of admissions based on clinical needs. Meets with families of prospective transfers for transitional care programs to explain the services of La Rabida. 10%
- Develop relationships with prospective referral sources. Identifies the needs of the referral source that La Rabida can meet. Identifies possible barriers to referrals and suggests solutions. Works with La Rabida marketing and clinical personnel to identify/evaluate/recommend program enhancements and new opportunities for programs at La Rabida. Participate in outreach activities, including those in the La Rabida marketing plan. 10%
- Provides a plan for introducing La Rabida to families of patients coming to La Rabida which may include tours and discussions with appropriate members of the healthcare team about expectations. 5%
- Communicate regularly with referral sources and families regarding the status of referrals. Facilitates communication with referrers on patient status after patients are transferred and periodically during hospitalization. 5%
- Works with Utilization Review Coordinator to ensure appropriate communications are made with third-party payers and admissions are approved. 3%
- Provide regular reports pertaining to referral patterns and timeliness of the referral process to the Chief Medical Officer, V.P. of Patient Care Services, and Senior Management. 3%
- Represents La Rabida in organizations/meetings related to La Rabida's mission and patient population, such as the Consortium for Medically Complex Children 2%
- Participates in appropriate quality improvement activities. 2%
- Requires travel to outside agencies as well as the ability to evaluate patients clinically. Additionally, extensive work may be done by telephone.
What we need from you:
- Education: Graduate of an accredited nursing program, BSN preferred
- Related Work Experience: Minimum of three (3) years in inpatient pediatric nursing, PICU, or NICU experience helpful
- Licensure/Certification: A current Illinois license as a Registered Nurse required
- Specialized Knowledge and Skills: Excellent clinical assessment skills, demonstrated knowledge of management of children with pediatric chronic conditions/disabilities. NICU experience preferred.
- CPR Required – only certifications issued by the American Heart Association, or the American Red Cross will be considered.
- Excellent written and oral communication skills
- Effective problem-solving
- Strong computer skills and knowledge of computer programs (MS Word, Excel, etc.)
