Now Hiring: Admissions Coordinator
Responsibilities:
- Serve as the primary point of contact for individuals seeking admission to our recuperative care facility.
- Conduct initial screenings and assessments to determine eligibility for admission.
- Coordinate with healthcare providers, social service agencies, and other stakeholders to facilitate referrals and admissions.
- Collect and review necessary documentation, including medical records and insurance information, to complete the admission process.
- Communicate effectively with residents and their support networks to address questions and concerns and provide assistance as needed.
- Collaborate with the interdisciplinary team to develop individualized care plans and ensure continuity of care.
- Maintain accurate and up-to-date admission records and documentation in accordance with regulatory requirements.
Qualifications:
- High school diploma or equivalent required; associate or bachelor's degree preferred.
- Compassionate and empathetic attitude towards individuals from diverse backgrounds, including the homeless population.
- Previous experience in a healthcare or customer service role preferred.
- Strong communication and interpersonal skills, with the ability to interact effectively with diverse populations.
- Excellent organizational skills and attention to detail.
- Ability to work independently and prioritize tasks in a fast-paced environment.
- Proficiency in computer skills, including Microsoft Office Suite and electronic medical records (EMR) systems.
- Compassionate and empathetic demeanor, with a commitment to providing patient-centered care.
