• Screening telephone calls for the Team Manager and facilitating communication between patients/families and other team members
• Managing daily schedules for all team members
• Attending weekly team meetings, preparing agendas, and following through on assignments
• Establishing, maintaining, and closing patient charts, including mailing and following up on required documentation
• Providing back-up documentation to the Billing Department as needed
• Ordering and tracking D.M.E. (Durable Medical Equipment)
• Entering patient care data into Vx and event tracking systems
• Communicating with staff and outside agencies as directed by the Team Manager
• Maintaining lists and schedules for part-time staff and ordering supplies
• Handling routine correspondence and processing patient care mail
• Maintaining A.D.T. (Admission, Discharge, Transfer) records and checking census daily for accuracy
• Covering for other patient care secretaries during absences

• Have a High School diploma or equivalent
• Possess three years of prior work experience (preferred)
• Have good typing skills and are computer literate
• Are able to handle multiple tasks simultaneously
• Demonstrate excellent communication and telephone skills
