TITLE: Activities Consultant
REPORTS TO: Psychosocial Coordinator
I. JOB SUMMARY
Provides support and formal consultation to activity programs. Under the direction of the Psychosocial Coordinator, provides consultation, training, and overall support for facilities’ activity programs and staff to ensure regulatory compliance and quality of services.
II. QUALIFICATIONS
A. Must possess a minimum of a bachelor’s degree.
B. Must possess active licensure/certification as a Certified Therapeutic Recreation Specialist (CTRS), Occupational Therapist Registered and Licensed (OTR/L), or a Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW).
C. Must have a minimum of 5 years experience working in a skilled nursing facility setting in activities or related field.
D. Must have a minimum of one year supervisory experience.
E. Must be able to read, write and speak the English language in an understandable manner.
F. Must posses proficient computer skills and experience working with an electronic medical record. Experience with Point Click Care preferred.
G. Must possess leadership, supervisor ability and willingness to work harmoniously with all personnel.
H. Must possess the ability to make independent decisions and to take directive action when circumstances warrant such action.
I. Must possess the ability to deal tactfully with facility personnel, residents, family members, visitors, consultants, government agencies/personnel, and the general public.
J. Must possess excellent communication abilities through listening, speaking and writing skills.
K. Must be able to maintain an appropriately professional manner at all times.
L. Must have patience, tact, a cheerful disposition and enthusiasm.
M. Must possess a valid Driver’s License, vehicle, and the willingness to travel within the City of Chicago, surrounding suburbs, Rockford, Wisconsin and other areas as designated.
III. PHYSICAL REQUIREMENTS
A. Must be able to move intermittently throughout the work day.
B. Must possess sight/hearing senses, or use prosthetics that will enable these senses to function adequately so that the requirements of the position can be fully met.
C. Must function independently, have flexibility, personal integrity and the ability to work effectively with the residents, family members, personnel and support agencies.
D. Must be in good general health and demonstrate emotional stability.
E. Must be able to cope with the mental and emotional stress of the position.
F. Must be able to relate to and work with (physically/mentally) ill, disabled, elderly, emotionally upset and at times hostile people.
G. Must be free of physical limitation.
H. May be required to lift equipment, supplies and occasionally move furniture.
I. Is subject to exposure to infectious waste, diseases, conditions, etc., including exposure to HIV, AIDS, and Hepatitis B virus.
IV. ESSENTIAL FUNCTIONS
A. Accepts and supports the Board of Directors philosophy of care, including the mission and values.
B. Assist with ensuring facility activity programs meet regulatory compliance and resident needs.
C. Assist with developing and maintaining relationships with community partners who support facility activity initiatives.
D. Conduct formal monthly or quarterly consultation to facility activity programs and provide documentation to the facility.
E. Hold facilities accountable for followup of items found/identified on monthly or quarterly consultation visits.
F. Participate in competency evaluations, skills fairs, and mock surveys as needed or requested.
G. Audit documentation (assessments, care plans, calendars, participation records, resident council, etc) to ensure compliance.
H. Conduct onboarding/new hire training and ongoing training needs as they arise with activity staff.
I. Assist facilities with survey readiness/survey preparation.
J. Attend and participate in Continuing Education Programs.
K. Ensure facility activity staff meets regulatory requirements for training and education.
L. Provide feedback on developing/adapting PCC related documents (assessments, care plan templates)
M. Assist with corporatewide and facility–specific inservice education as needed in the area of programming.
N. Communicate any updates/progress/concerns with facility leadership/Administrator.
O. Perform other related duties as assigned.
