Position Title: Account Executive (Liaison, Hospice, SNF) - HomeCare

Job Summary: We are seeking a dynamic Account Executive to serve as a liaison between our hospice services and skilled nursing facilities (SNF). This role involves building and maintaining relationships with healthcare providers to enhance patient care and service delivery.

• Responsibilities:
  • Develop and maintain partnerships with SNFs and healthcare professionals.
  • Educate providers on hospice services and patient eligibility.
  • Conduct regular visits to facilities to ensure quality service and address concerns.
  • Collaborate with internal teams to streamline patient transitions.
• Qualifications:
  • Bachelor’s degree in healthcare or related field.
  • Proven experience in healthcare sales or account management.
  • Strong communication and interpersonal skills.
