Develop and maintain relationships with healthcare providers and facilities.

Educate staff on hospice services and patient care options.

Collaborate with interdisciplinary teams to ensure seamless patient transitions.

Conduct regular visits to SNFs to assess needs and provide support.

Track and report on account performance and growth opportunities.

Qualifications:

Experience in healthcare sales or account management.

Strong communication and interpersonal skills.
