Hours: Full Time; 40 Hours: Monday - Friday: 8:00 am - 5:00 pm

Annual Wellness Visits
- Conduct initial and subsequent Annual Wellness Visits (AWVs) for patients in the office, ensuring compliance with Medicare guidelines.
- Perform patient health risk assessments and document findings in the electronic health record (EHR).
- Provide personalized health education based on assessment results.
Preventive and Quality Care
- Educate patients on age-appropriate screenings, vaccines, and other preventive care measures.
- Assist in identifying and addressing gaps in care to meet quality measures, such as HEDIS and other value-based metrics.
- Monitor and follow up on overdue screenings and chronic disease management goals.
Care Coordination
- Collaborate with providers to coordinate disease management plans and promote effective patient care.
- Facilitate patient referrals and ensure accurate documentation and follow-through.
Administrative Duties
- Process and follow up on medical prior authorizations for medications, tests, and procedures.
- Manage incoming and outgoing referrals, ensuring proper communication and tracking.
- Maintain accurate patient records and documentation per organizational policies.
Patient Education
- Provide guidance and educational resources to patients regarding chronic disease management, lifestyle modifications, and preventative care.
- Engage patients in discussions to empower their active participation in their healthcare journey.
Other Responsibilities
- Participate in continuous learning and training to stay updated on wellness visit guidelines, quality measures, and practice protocols.
- Collaborate with the clinical team to improve workflows and optimize patient care delivery.
- Assist with other duties as assigned to support the practice.
- Completion of a Medical Assistant program (certification preferred but not required).
- Experience performing Annual Wellness Visits or willingness to learn.
- Will train on Medicare guidelines, preventive care measures, and quality initiatives (e.g., HEDIS).
- Strong organizational skills and attention to detail.
- Ability to effectively communicate with patients and providers.
- Proficiency in electronic health records (EHR) systems.
