Growing together.** The Senior Community Care (SCC) program is a longitudinal, integrated care delivery program that coordinates the delivery and provision of clinical care of members in a facility setting. The APC within SCC provides care to our highest-risk health plan members and is part of an interdisciplinary team. **Primary Responsibilities:** + Provide clinical coverage for the primary APC during periods of PTO or short/long term absences + Manage members through an acute medical situation or an exacerbation of a chronic condition, including testing, diagnostics, and interventions + Manage members during a short term, sub-acute rehabilitation episode + Conduct comprehensive, monthly/routine, and/or acute care visit assessments + Effectively manage acute and chronic medical and behavioral conditions + Ensure accurate and complete documentation, including ICD 10 conditions + Communicate and collaborate with the interdisciplinary care team You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in. **Required Qualifications:** + Active NP or PA license or ability to obtain by start date. Licensure must be unencumbered, free of any open/unresolved disciplinary actions including probation or restrictions against privilege to practice + For NP: Active ANCC or AANP national certification in Family, Adult, Adult-Gerontology Primary Care, etc. Licensure must be unencumbered, free of any open/unresolved disciplinary actions including probation or restrictions against privilege to practice + Current, active DEA licensure/prescriptive authority or eligibility to obtain post-hire, per state regulations (unless prohibited in state of practice) + 1+ years of clinical/family/geriatric practice experience as an NP or PA + Willingness to travel via car, airplane, etc. + Willingness to obtain additional licensure in assigned geographic areas + Ability to complete physical requirements of the job including lifting a 30-pound bag in and out of the car, navigating stairs and a variety of dwelling conditions, and sit, stand, and kneel to perform physical assessment + Proficient with electronic medical records and the internet **Preferred Qualifications:** + Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs + Understanding of Geriatrics and Chronic Illness **Washington Residents Only:** The salary range for this role is $88,000 to $173,200 annually. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
