About The Position
**Job Title:** Registered Nurse (RN) **Job Specialty:** Acute Rehab **Job Duration:** 9 months **Shift:** Night Shift; 7:00 PM - 7:30 AM, 3x12 hours **Guaranteed Hours:** 36 hours per week **Experience:** Minimum three years of relevant nursing experience **License:** State RN License in good standing **Certifications:** BLS (American Heart Association) **Must-Have:** - Relevant nursing experience - Patient care skills - Therapy and documentation knowledge **Job Description:** - Assume responsibility for a group of patients for a designated timeframe and provide care using nursing processes, trends, and technology. - Perform all responsibilities and duties specific to the scope of service required by the unit. - Implement procedures and theories related to the specific area of practice. - Maintain up-to-date and accurate documentation of nursing care to ensure information integration for healthcare team use. - Complete initial assessment upon patient admission and develop an appropriate plan of care in accordance with unit standards based on patient and family needs. - Implement plan of care, nursing interventions, and patient care procedures. - Modify the plan of care based upon continuous evaluation. - Take responsibility for patient and employee safety by reporting and/or correcting safety hazards and incidents. - Communicate with others directly and privately when necessary to resolve issues. - Offer constructive feedback to assist in the professional development of peers and confers with colleagues to expand the knowledge base. - Identify learning needs and take initiative to begin appropriate solutions. - Fulfill all requirements of departmental competencies. **Department Specific Job Functions:** - Complete initial assessment upon patient admission to the rehab unit and develop an appropriate rehab plan of care. - Implement rehab plan of care and coordinate nursing activities with the interdisciplinary rehabilitation team. - Share information on disease processes and teach nursing techniques to help rehab patients and families develop self-care skills. - Participate in rehab rounds and team meetings. - Provide ongoing patient education and reinforce teaching done by therapy in rehab. - Possess specialized knowledge and clinical skills necessary for care of individuals with physical disabilities and chronic illness.
