About The Position
**Job Title:** Registered Nurse - Case Manager **Job Specialty:** Home-Hospice Care Operations **Job Duration:** 13 weeks **Shift:** 8-hour Day Shift (40 hours per week) **Guaranteed Hours:** [Not specified] **Experience:** Minimum 1 year of Home Health RN experience **License:** Active RN license in Illinois **Certifications:** Active BLS (AHA) **Must-Have:** - Wound care experience, including wound vacs, pleural drains, and ostomy care - Ability to perform peripheral blood draws, central line care, and IV administration through multiple devices (elastomeric, IV pumps) - Excellent assessment skills - Experience with computer documentation - Reliable transportation, active driver’s license, and insurance **Job Description:** - Manage a caseload of 20-30 patients per week, with individuals typically having multiple comorbidities such as CHF, COPD, pneumonia, cancer, dementia, kidney disease, and more. - Provide comprehensive care including post-surgical care, wound and ostomy care, lab work, and IV infusion. - Utilize computer charting systems and Sapphire IV pumps effectively. - Document patient information thoroughly on provided laptops. - Wear any solid color scrubs as appropriate attire. - Travel required within designated zip codes for patient visits. **Coverage Area:** - East to West: Mountgreenwood to Willow Springs - North to South: Burbank to Chicago Ridge - Zip codes: 60459, 60415, 60455, 60453, 60456, 60457, 60458, 60480, 60652, 60655, 60805, 60499, 60501
