StartDate: ASAP Available Shifts: 8 D Pay Rate: $1423.67 - $1559.98
How long is the contract assignment? 13 weeks 2) Schedule of assignment hours? M – F, 8am – 5pm 3) Will this candidate be visiting patients in care facilities or only in patient’s homes? Facilities and homes 4) Will you want to hire the contractor at the end of the contract or is this truly a contract assignment? Will want to hire if appropriate 6) What region are you in? Home health or Hospice? Home health 7) What’s creating the request? - work load 8) To which location will this person report for assignment, full address, Including the OU (Location Code) 8316 College park: 2314 Sullivan Rd Suite 120, College Park Ga 30337 Please confirm above location is correct and list any other territories that are to be covered by the candidates: 9) How many miles per week will the consultant travel? 30/40 a day?
