About The Position
Job Title: Registered Nurse Job Specialty: Case Management (Utilization Review) Job Duration: 13 weeks Shift: 8H Days, Monday to Friday Guaranteed Hours: 37.5 hours per week Experience: Minimum of 1 year of inpatient case management experience License: License required to submit Certifications: BLS, PRI (Patient Review Instrument) Certification, SCREEN Training Certification Must-Have: Knowledge of discharge planning and making referrals, and ability to function independently and collaboratively within an interdisciplinary team for discharge planning Job Description: - Knowledge of discharge planning and making referrals; will typically work on a Medical-Surgical or Telemetry unit - Collaborate with a Social Worker to manage floors with 33-40 patients; individual caseloads can range from 12-25 patients - Ensure effective discharge planning in inpatient settings; plan for patient discharge to home, community, or skilled nursing facility - Handle various patient coordination tasks such as obtaining housing, preparing shelter packets for homelessness, and handling hemodialysis packets - Use of Electronic Medical Records (EMR) system Allscripts; preferred knowledge of CarePort, MCG, and/or InterQual systems - Prefer some experience in Utilization Review - Required to participate in hospital orientation and EMR training for 2 days, followed by 1-2 weeks with a preceptor - Occasionally work on weekends and holidays with manager approval; may be required to cover multiple units during these times - Follow business casual dress code or any color scrubs except Royal Blue or Maroon; lab coat provided - Management supportive of taking a full lunch break; breaks according to facility policy Preference for candidates with a positive attitude, critical thinking ability, and good time management skills.
