JOB DEFINITION:
The Team Leader of the Critical Time Intervention Team (CTI) of Nassau and/or Suffolk County provides administrative, clinical and operational oversight to the team. The Team Leader provides supervision to the staff, conducts team meetings, manages relationships with the hospitals and promotes model fidelity.
EDUCATIONAL REQUIREMENTS:
Licensed Clinical/Masters Social Worker, Licensed Marriage and Family Therapist, Licensed Mental Health Counselor, or Licensed Creative Arts Therapist
EXPERIENCE REQUIRED:
- A minimum of three (3) to five (5) years post licensure experience working with individuals who have mental health, or co-occurring mental health and substance use disorders.
- At least 1 year of supervisory experience.
- Ability to make data- driven decisions.
- Ability to coordinate responsibilities. Excellent organizational and time management skills required.
- Working knowledge of computer software and electronic health record systems.
- Demonstrated competency in written, verbal and computational skills to present and document records in accordance with program standards.
- Exceptional public speaking and presentation skills.
- Demonstrated competency in development and implementation of the interdisciplinary planning process.
- Exceptional interpersonal communication and team collaboration skills.
- Crisis intervention and emergency problem solving competency and experience.
DUTIES AND RESPONSIBILITIES:
- Provide supervision to all staff as directed. All supervisions are required to be documented.
- Complete eligibility assessments within the community/field.
- Conduct weekly team meetings with the entire team whether virtual or in person.
- Manages relationships with hospitals and other referral sources.
- Ensures and reinforces model fidelity with the team.
- Ensures continuity between fieldwork and meetings. Team leaders may need to cover for each other or their staff. This includes all services provided.
- Reviews entire caseloads and ensure caseload size meets CTI guidance.
- Oversees and manages staff productivity and staff wellness.
- Monitors quality of documentation by regulatory bodies and agency policies.
- Reinforces all agency and programmatic policies.
- Monitors phases of program based on CTI guidance.
- Assists in covering caseloads when needed.
- Complete all required training and monitors staffs' compliance with training
- Attendance at all required meetings whether internal or external.
- Oversee and manage all aspects of service delivery to ensure quality of care and compliance with all agency and governmental policies and standards.
- Participate in recruitment, hiring, training and ongoing evaluation of staff under supervision from the Program Director.
- Create, maintain and review staff schedules as needed.
- Participate in data informed, outcome driven, planning and execution of new program initiatives.
- Support the use of data in clinical supervision to inform decision-making
- Identify and implement quality improvement trainings and activities as part of professional development goals
