At Custom Home Health, we're seeking a dedicated Team Coordinator to manage administrative tasks, coordinate care, and play a vital role within our referral management and intake department. You'll ensure efficient scheduling, patient communication, and contribute to the overall excellence in patient care.
Schedule: Monday - Friday, 8:30 AM - 5:00 PM

Essential Job Functions/Responsibilities:
Manage patient scheduling for clinical teams, ensuring proper care coordination and follow-up.
Answer phones and provide high-level customer service to patients, families, and healthcare providers.
Handle incoming referrals, scheduling registration visits, and coordinating admission assessments.
Maintain accurate records, logs, and patient information in the EMR system.
Assist in verifying patient insurance information and ensuring complete referral documentation.
Participate in weekend and after-hours on-call rotations to cover phone lines and emergency scheduling.
Provide clerical support to the referral management and intake department, including maintaining schedules and tracking patient progress.
Proactively collaborate with the referral team, clinical staff, and external healthcare providers to ensure smooth transitions for patients.
Position Qualifications:
High school diploma required; college degree preferred.
Proficiency in Microsoft Office.
Strong organizational, multitasking, and time management skills.
Excellent customer service and telephone communication abilities.
Flexible, patient, and cooperative, with a strong work ethic.
Experience in healthcare or referral management is a plus.
