A premier healthcare facility is a not-for-profit long- and short-term care facility providing services to the disabled and elderly for over 40 years located in Brooklyn, NY is seeking a TTP Clinical Coordinator
Type: HC Contract
Schedule: Full-Time, M-F:09:00am-05:00pm or 08:00am-04:00pm
Location: Brooklyn, NY
Job Functions and Responsibilities:
Mail/Fax/Email MD orders to patient's physician
Request signed MD orders, letters of medical necessity, progress notes from MD offices
Maintain a positive professional relationship with various MD offices to ensure timely receipt of orders, letters and progress note for patient care.
Refer patient's to Visiting Doctor Services and follow up to ensure patient was seen and documentation received timely
Receive/Scan patient's assignment of benefits into EMR system to ensure compliance with federal, state and city regulations
Ensures that accurate admission and discharge data information is entered in the system
Inputs MD orders/Face-to-Face certifications into tracking
Tracks and follow up on outstanding MD orders/Face-to Face certifications
Answers and Triages CHHA phone calls
Email/Faxes notes to MLTCs
Order medical supplies for patients as requested by the RN and nursing supervisor
Assists CHHA Operations Manager to run the office operations, including ordering supplies, scheduling visits, filing and so on.
Participates in Performance Improvement projects
Accepts and completes assignments as given within scheduled time frame
Projects positive business-like demeanor. Promotes positive work environment
Maintains confidentiality of patients' records
Accountable maintains an acceptable records of attendance
Complies with policies and guidelines established for meal periods, breaks, sick calls, requests for time off and dress code
Qualifications:
High school diploma
Associate Degree preferred
Good communication skills
Good Math skills
Good typing skills
Experience working in a computerized environment
Absolute integrity and sufficient maturity to handle confidential aspects of work
Physical Requirements:
Posses the sight/hearing senses or uses prosthetics that will enable theses senses to function adequately so that the requirements of this position can be fully met
Must comply with the regulatory health status requirements of the NY State Department of Health
Work Requirements:
Work in the office environment with regular exposure to Administration, Management, office staff, vendor agency's personnel, patients and their significant others.
Must be able to sit for prolong period of time, transport materials or equipment, lift, move or carry some items throughout the day
Professional Requirements:
Ability to set priorities, make logical decision and solve problems
Effective communication skills
Ability to evaluate and improve operational processes
Ability to function independently
Ability to relate and work with others within each level of authority
Knowledge in cultural sensitivity and awareness
Knowledge in utilizing principals of "customer Service: in all areas of job responsibility
Ability to seek guidance when applicable
