Job Description Summary
Student pharmacist in any year of Pharmacy school that has been trained through MUSC MTM Center protocols and OutcomesMTM protocols and is supervised by a licensed pharmacist.Provides medication therapy management (MTM) services via outbound calls and possibly a triage of inbound calls to the MTM center, monitors eFax for prescriber responses and completion of failed faxes, supports the pharmacist for completion of Comprehensive Medication Reviews (CMRs) through pre-work up and case preparation, communicates with network pharmacies, prescribers, and beneficiaries to provide MTM services.
Additionally, may be called upon to assist the MTM Executive Director in the training of pharmacy externs rotating through the MTM Center.

Job Description
35% Deliver Medication Therapy Management Services
• Performs medication reviews to identify medication related problems; document MTM claims.
• Collect a complete medication list from the patient (OTCs, herbals, and prescription medications).
• Performs interactive consultations and makes recommendations/interventions for members and providers, appropriately documents all interventions in EPIC.
35% Provider and Patient Support
• Generate and send prescriber communications; distribute and monitor communications.
• Answer phones and check voicemail messages as needed.
• Provide excellent customer service, responding to customer complaints and questions in an efficient and courteous manner while including the pharmacist in relevant communications.
• Documents and records services/activities in appropriate databases/software to ensure compliance.
• Continues seeking higher MTM certification credentials.
20% Manage MTM Opportunities
• Supports the efforts of the clinical team through collaboration with prescribers and targeted beneficiaries in order to ensure appropriate, evidence-based drug selection and resolve medication-related problems.
• Reviews MTM Opportunities queue regularly to ensure staff respond to all TIPs as well as CMR opportunities.
• Schedules MTM encounters, patient engagement, and case management.
• Schedule follow-up appointments as needed for additional interventions identified during the CMR and set reminders for follow up with patients or prescribers.
10% MTM Outcomes
• Supports basic data analytic activities and tracking of interventions and assists with preparation and documentation of responses to regulatory audits as needed.
