Job Summary:
We are seeking a dedicated and compassionate Speech Therapist, and provide essential therapy services to patients within a residential setting. As a Speech Therapist, you will be responsible for assessing, planning, and implementing rehabilitation services for speech, language, and/or swallowing disorders as prescribed by the attending physician. You will also provide speech therapy consultations to Agency staff and the patient’s physician as needed. This position requires regular travel for patient visits in varied residential environments based on an assigned caseload.
Key Responsibilities:
Assess, plan, and implement individualized rehabilitation services for patients with speech, language, and/or swallowing disorders.
Perform diagnostic testing to evaluate speech and language abilities, establishing goals and prognosis for recovery.
Document and report patient progress in treatment, including any changes in condition, to the physician.
Instruct family members and healthcare team members on supporting patients to improve communication skills.
Re-evaluate patient progress and adjust rehabilitation programs as necessary.
Prepare and submit periodic reports to the physician and third-party payers detailing care provided.
Make referrals to outside clinics or community services as appropriate.
Maintain accurate clinical and progress notes, submitting required paperwork in a timely manner.
Coordinate with other community agencies for follow-up care and support.
Participate in in-service training, case conferences, and staff meetings.
Maintain professional competency through continuous education and in-service attendance.
Perform other duties as assigned by the supervisor.
Schedule and Shift Details:
Flexible work schedule based on an assigned caseload.
Regular travel required to attend meetings and conduct patient visits in various residential homes.
Travel:
Daily travel required for meetings and visits to residential homes.
Qualifications:
Master’s degree in Speech Therapy or a related field.
Minimum of 1 year supervised post-certification experience in an institutional setting.
Valid Speech-Language Pathology (SLP) license in the state of Illinois (IL).
Current certifications in Basic Life Support (AHA BLS), Advanced Cardiac Life Support (AHA ACLS), and/or Pediatric Advanced Life Support (AHA PALS) are required.
Prior experience with Residential Home Health Hospice is not a requirement.
Preferred Qualifications:
SLP License Number for verification.
