Service Coordination position - White Glove Community Care

White Glove- the king in Early Intervention service provision is now expanding its teams in service coordination!

The Service Coordinator is the designated primary contact between service providers and parents. They also help families understand their rights in the NYS Early Intervention program and available services to address the concerns they have for their children.

It can be your side, primary or part time job!

Qualifications:

Qualified applicants are those with:

-1+ years of experience in service coordination and early intervention.

-Knowledgeable with Early Intervention Program systems.

-Must have excellent written and oral communication skills.

-Be efficient, well organized, and able to manage multi-family case load.

-Strong commitment to helping families. >Bilingual a plus!

Job description:

-Assist families to plan and obtain services appropriate to their needs.

-Supervise, Coordinate and monitor delivery of identified services and the proper use of resources.

-Participate in the development, review and evaluation of individualized family service plans (IFSP).

-Participates with the family in the IFSP meeting to finalize services that will be provided.

-Document, audit & maintain written records and reports regarding service coordination activities.

-Is instrumental in developing caseloads delegating and providing services as authorized to children and their families.

-Attend and participate in continuing education, ongoing staff in-services and quarterly meetings.

-Follows mandatory guidelines according to EI Regional Policies and Procedures and Employee Handbook.
