Position Summary:
The Home Care Manager is responsible for overseeing the delivery of high-quality, compassionate, and personalized care to residents within our senior living facility. This position involves managing home care services for residents, ensuring that their needs are met with the utmost care, dignity, and professionalism. The Home Care Manager will supervise a team of home care aides, coordinate with medical staff, and ensure compliance with all regulatory and safety standards.
Key Responsibilities:
Care Management & Coordination:
- Develop individualized care plans for residents based on assessments, ensuring they meet both medical and personal care needs.
- Monitor and assess residents' physical, emotional, and social well-being, making adjustments to care plans as needed.
- Coordinate with healthcare professionals, including physicians, nurses, and therapists, to ensure comprehensive care delivery.
- Act as the primary point of contact for residents and their families, addressing concerns and providing updates on resident progress.
Supervision & Staff Management:
- Oversee and lead a team of home care aides and nursing assistants, providing guidance, training, and performance evaluations.
- Schedule and manage staffing to ensure adequate coverage for all shifts and that residents’ needs are consistently met.
- Foster a collaborative and supportive team environment, encouraging ongoing professional development.
Regulatory Compliance & Quality Assurance:
- Ensure that home care services comply with all local, state, and federal regulations, as well as facility policies and procedures.
- Conduct regular audits and assessments to ensure the highest quality of care is being delivered.
- Participate in care reviews, ensuring that all documentation is accurate, timely, and thorough.
Family & Resident Communication:
- Serve as the key liaison between the facility and the residents' families, ensuring transparent and effective communication regarding care plans and any changes in health status.
- Provide support to families, offering resources and guidance when necessary to help them navigate the care process.
Training & Development:
- Implement training programs for home care staff, ensuring they are well-versed in current caregiving best practices, emergency protocols, and infection control procedures.
- Regularly evaluate the performance of care staff, providing constructive feedback and coaching for improvement.
Resident Advocacy & Emotional Support:
- Advocate for the rights and preferences of residents, ensuring that their dignity is upheld throughout all aspects of their care.
- Provide emotional and social support to residents, ensuring they feel safe, respected, and valued within the facility.
Budgeting & Resource Management:
- Assist with the budgetary management of the home care department, ensuring effective use of resources, supplies, and staff time.
- Help identify and implement cost-saving strategies without compromising the quality of care.
Qualifications:
Education:
- Bachelor’s degree in Nursing (Preferred)
Experience:
- At least 3 years of experience in home care or senior living management.
- Experience working with the elderly population, particularly those with chronic conditions or cognitive impairments, is highly preferred.
Skills & Abilities:
- Strong leadership, communication, and interpersonal skills.
- Excellent organizational and problem-solving abilities.
- Ability to handle sensitive situations with empathy and discretion.
- Proficient in care plan development, assessment, and documentation.
- Knowledge of regulatory requirements in senior care and home care services.
- Ability to work collaboratively in a multidisciplinary team environment.
Physical Demands:
- Ability to lift and assist residents as needed (up to 50 lbs).
- Ability to stand, walk, and move throughout the facility during work hours.
- Must be able to respond to emergency situations in a timely manner.
Work Environment:
- The Home Care Manager will work in a senior living facility, which may include exposure to various health conditions. Proper personal protective equipment (PPE) will be provided as needed to ensure the safety of all employees and residents.
